Like other Khanid vessels, the Sacrilege fuses Amarr armor tanking with Caldari missile weapons, providing a rare combination which is difficult to train for: Amarr pilots will generally prefer to fly a Zealot since they will already have strong energy turret skills, while Caldari pilots will need to train up armor tanking. However, the Sacrilege is a capable ship, powerful in the hands of well-trained pilots.

In terms of hull bonuses, the Sacrilege is most comparable to a Cerberus. While the Cerberus is designed to be active tanked while using long range HAMs, the Sacrilege is designed to use heavy missiles and be passive tanked. The Sacrilege is slightly slower but has more drone capability. 
With the 2022 HAC Rebalancing, the Sacrilege has replaced the Cerberus as the preeminent missile boat for large scale bloc warfare in nulsec.

The Sacrilege performs admirably whether flown solo or in a fleet, able to fit either dual reps to tank as much damage as it dishes out or a buffer tank to maximize survivability when primaried. Notably, Heavy Assault Missile Legion fits strongly resemble those for the Sacrilege in much the same way a Heavy Missile Tengu represents a more powerful Cerberus, so depending on a player's goals, skills, and budget they may opt to fly a Legion instead.

Due to the ubiquity of brawling armor fleets in wormhole space, the Sacrilege is one of few options for a missile boat suitable for a typical WH fleet, alongside Damnation and Legion. It is also perfectly suited to armor-tanked Heavy Assault Cruiser fleets, particularly by offering greater capability for close-range combat than the Zealot while still hitting out to a respectable range with HAMs.

Sacrilege is one of few hulls that perform well in Dark Abyssal Deadspace. Its armor tank coupled with Afterburner, that works in Dark Abyss, provide bonus of low signature radius married with velocity still high. In many cases when you meet a Sacrilege in highsec, in all likelihood, it will be an Abyss diver.
